Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) by not securing the facility and allowing them to elope. The facility had installed a tracking device in the AV’s shoes due to exit seeking behavior. On or about May 12, 2024, the AV was unable to be located in the facility during a med pass. the AV’s tracking system was turned on and the facility was able to locate the AV approximately three blocks away from the facility, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00806 $188.00 fine assessed
