Findings: The Alleged Victim (AV) and Witness #1 IW1) have a history of altercations. AV likes to wander into W1's room and go through their items, and W1 doesn't like it. On or about May 13, 2024, AV wandered into W1's room, W1 tried to remove AV from his/her room and AV wasn't leaving, in response W1 hit AV and AV scratched W1. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
