Findings: Alleged Victim (AV) has a history of wandering and/or elopement, AV is also known to have exit seeking behaviors. On or about May 14, 2024, at approximately 2:45 PM, AV was returned to the facility by an unknown person who had found AV walking out in the community. Staff where unaware when AV left or how long AV was gone from the facility. The facility failed to provide a safe environment for AV, putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-01177 $250.00 fine assessed
