Findings: The Alleged Victim (AV) is a known fall risk due to vision impairment and has suffered multiple falls from March 2024 until May 2024. On or about May 16, 2024, AV was returning from the bathroom and suffered a fall, causing a skin tear to his/her knee. Interventions in place were not effective to keep AV from falling. The facility's failure to properly care plan to reduce the risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00221 $1000.00 fine assessed
