Findings: Witness 1 (W1) and Alleged Victim (AV) have a known history of being involved in other resident-to-resident altercations. On or about May 16, 2024, W1 walked into AV’s apartment. AV stopped W1 at the doorway and when W1 went to leave, AV grabbed a metal paper towel holder and hit W1 in the face. W1 then pushed AV knocking AV to the ground. W1 and AV have had previous altercations on or about March 20th and 26th, 2024. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
