Findings: Alleged Victim (AV) is care planned to be in toileted every morning, before and after each meal, and at night before bed plus two (2) additional checks. Staff are to check on AV four (4) times per shift plus (three) 3 additional checks per shift for cleanliness. On or about April 12, 2024, AV was found with feces all over AV’s legs. On or about May 12, 2024, AV had once again been found multiple times with feces/urine all over the floor. The facility failed to follow AV’s service plan regarding his/her cleanliness, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00984 $500.00 fine assessed
