Findings: Alleged Victim (AV) relies on the facility for his/her care needs. AV has ongoing skin issues, and the facility is aware of the issue. AV has scheduled toileting, and staff are to reposition AV every two (2) hours for skin integrity. AV has been found in wet briefs and is experiencing skin breakdown. AV has refused to be repositioned and other care, however, his/her service plan does not identify or provide instructions for caregivers when AV refused to be repositioned or refuses overall care. The facility failed to properly plan care around AV’s refusal to care, leading AV to sustain skin breakdown, which is a violation of resident rights, is neglect of care and constitutes abuse.
