Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV was care planned for assistance when transferring to and from bed in the morning and evening. According to an investigation, on or about May 18, 2024, AV pressed his/her call light for assistance to get out of bed, which went unanswered due to lack of staffing, resulting in AV transferring AV’s self to go to breakfast independently. It was also discovered that AV was not receive walks when requested due to lack of staffing. The facility failed to provide appropriate services, posing a serious risk of harm to AV, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: ALFCP25-00007 $250.00 fine assessed
