Findings: On the evening of May 20, 2024, Alleged Perpetrator 2 (AP2) responded to Alleged Victim's (AV) call light and AV requested assistance with toileting and thorough peri care. While AP2 was assisting AV with his/her Activities of Daily Living, AV became upset with AP2, causing AP2 to become "stressed" in which AP2 left AV's room, slamming the door of AV's apartment while exiting AV's room. Incident between AP2 and AV occurred around 4:50pm. When AV's private caregiver arrived at the facility around 9:50pm, AV's private caregiver and facility staff found that AV's door was jammed and unable to be opened for some time; 911 was contacted while staff worked to, and were eventually able to, open the door. As a result, AV did not receive nightly care in a timely manner. Based on facility documentation and interviews, AV was upset by the incident and suffered emotional harm. There is sufficient information to indicate that AP2's actions of slamming AV's door on the evening of May 20, 2024, did cause AV significant emotional harm. AP2's action is considered verbal/emotional abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
