Findings: According to the documentation, the facility failed to follow the care plan by ensuring the implementation of interventions for the Alleged Victim’s (AV) know fall risk. On or about May 18, 2024, the AV was found on the floor in their bathroom with a wound to their head. The AV was sent to the hospital for the head wound where they received approximately nine stitches. During the investigations, it was found the chime alarm for the AV was off during the time of this incident and the care plan for the AV says it needs to be on, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00782 $1125.00 fine assessed
