Findings: AV and W1 have history of being in resident-to-resident altercations with other residents and each other. AV has a history of wandering the facility and wandering into other resident rooms. On or about May 22, 2024 AV went into W1's room, which upset W1 leading to a resident-to-resident altercation between AV and W1. W1 has an intervention in place that if staff see W1's door open, they are to offer to shut it to help prevent people from wandering in. AV has no interventions in place in place to mitigate AV from wandering into other residents rooms. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s ongoing wandering into other resident rooms and prevent resident-to-resident altercation between AV and W1, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
