Findings: Witness #1 (W1) is known to engage in altercations and has a one to one caregiver. On or about May 23, 2024, the Alleged Victim (AV) walked into another residents room. W1 was sitting on a bench near the room and saw AV and jumped up quickly after AV and hit him/her. Staff were completing shift change and W1 was sitting right with staff, however, they could not stop W1 in time. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
