Findings: The Alleged Victim (AV) relies on the facility for care. According to an investigation, AV had just received a treatment which is known to cause AV itching skin, but there were no skin care interventions put in place to monitor AV for itchiness. On or about May 25, 2024, AV was found with multiple scratches on their shoulder and hands. The facility failed to implement interventions and/or appropriately care plan for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
