Findings: The Alleged Victim (AV) and Witness 1 (W1) both rely on the facility for their care and have a history of physical altercations. According to an investigation, AV and W1 have been in multiple physical altercations between approximately May 14, 2024 and May 27, 2024, some resulting in injury to include AV being thrown against the wall. The facility failed to implement interventions and/or appropriately care plan for AV and W1's known behaviors and provide a safe environment for AV, causing repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
