- Findings
- The Alleged Victim (AV) relies on the facility for his/her care. Between February 21, 2024 and May 19, 2024 AV had multiple documented skin tears. The facility failed to implement interventions and/or appropriately care plan for AV’s repeated skin tears and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
