Findings: The Alleged Victim (AV) relies on the facility for his/her care. The AV is care planned for staff to ensure AV ambulates with his/her walker at all times. According to an investigation, on or about May 23, 2024, AV was following Witness 1 (W1) to another room, when W1 saw AV following closely and began moving quickly to close the door before AV arrived, resulting in AV’s finger getting shut in the door. AV’s finger was red then turned purple in color. Later in the day, AV was walking fast in the hallway and tripped on the carpet and fell forward. AV was not using his/her walker during either incident. The facility failed to follow AV’s care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: ALFCP25-00013 $250.00 fine assessed
