Findings: The Alleged Victim (AV) and Witness #1 (W1) have histories of altercations with other residents and each other. On or about May 28, 2024, AV and W1 engaged in an unwitnessed altercation where AV suffered a bruise to his/her head and W1 suffered redness and scratches to his/her face and neck. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
