Findings
Alleged Victim (AV) Service Plan dated on or around February 28, 2024, Behavior Management: Triggers for AV include others taking items from AV, redirect AV if AV is approaching other residents, redirect AV with a walk or a snack. WItness 1 (W1) Service Plan dated on or around March 30, 2024, Behavior Management: people being in W1's space can trigger behaviours including hitting and yelling, saff are to try and redirect W1 if they see behaviors. AV and W1 were involved in a prior resident-to-resident altercation on May 16, 2024, where AV and W1 struck each other in the hall. On or about June 1, 2024 AV and W1 were involved in a resident-to-resident altercation W1 and AV were sitting near each other in the dining room. AV tried to take cake from W1, W1 grabbed AV's arm and then struck AV in the arm, AV then struck W1 in the face. Interventions were not being followed at time of incident for AV or W1. The facility failed to follow the care plan of AV and W1 resulting in a resident-to-resident altercation.
