Findings: Witness 1 (W1) has history of resident-to-resident altercations. W1 service plan dated on or around February 28, 2024 indicates if staff see W1 approaching other residents they are to help redirect h/h to a common area and offer a snack. If staff see W2 approaching (other resident) from behind they are to redirect W1. If staff see W1 approaching other residents trying to touch them they are to redirect h/h and take h/h for a walk. W1 is frequently disoriented, even in familiar surroundings requires supervision. On or about June 1, 2024 W1 entered Alleged Victim (AV) private room and AV told W1 to leave. W1 struck AV in the face with open hand, resulting in AV having fear, and feeling unsafe and being afraid of W1. AV was reported as crying and being upset after the incident. The facility failed to follow W1 care plan resulting in a resident-to resident altercation.
