Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. On or about May 28, 2024, W1 had been involved in an altercation and staff were instructed to keep W1 away from others and to not leave W1 unsupervised in the common area. On or about May 29, 2024, staff heard screaming and ran to the common area to find W1 lying on the floor in front of AV. AV had suffered a skin tear to his/her right hand. The facility's failure to ensure supervision of W1 is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01343 $500.00 fine assessed
