Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. On or about May 28, 2024, AV had been involved in an altercation and staff were instructed to keep AV away from others and to not leave AV unsupervised in the common area. On or about May 29, 2024, staff heard screaming and ran to the common area to find AV lying on the floor in front of W1. AV had suffered a lumbar strain and a bruise to his/her right knee. The facility's failure to ensure supervision of AV to prevent him/her from falling is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01347 $500.00 fine assessed
