- Findings
- The Alleged Victim (AV) is a fall risk with a history of falls. According to an investigation, on or about June 2, 2024, the AV was found on the floor, resulting in a wrist injury. The facility failed to appropriately care plan for the AV's known fall risk, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
