According to an investigation, on or about May 29, 2024, AV was found to have a large, dried, mucous mass in the back of h/h mouth that required staff to dislodge it. The facility failed to provide oral hygiene services for the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
