Findings: The Alleged Victim (AV) relies on the facility for his/her care, has a history of numerous repeated falls. On or about June 12, 2024, AV's spouse found AV on the ground in the outside courtyard. AV's slippers had come off either before or after the fall, and there were scratches on the left side of AV's face. The facility failed to implement interventions and/or appropriately care plan related to AV’s safety and history of falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
