Findings: According to the documentation, the facility failed to ensure a safe environment for the Alleged Victim (AV) who resides in a secured memory care facility. On or about April 12, 2024, the AV was seen by a visitor outside the secured unit by themselves. When staff checked on the resident, they appeared to be tired and needed a wheelchair to get back into the facility. It was later determined a locked safety door was used and was not locked allowing anyone to exit through this door, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
