Findings: Alleged Victim (AV) has a history of physical aggressive behaviors. On or about June 28, 2024, witness 1 (W1) and AV were involved in a second physical altercation where AV called W1 “stupid”, and it led to W1 and AV hitting each other. As a result, W1 was afraid to sleep in W1’s room with AV after the altercation on June 18, 2024. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
