Findings: Both the Alleged Victim (AV) and Witness 1 (W1) rely on the facility for care. W1 has a history of getting overstimulated which results in aggression and is care planned to be monitored when around other residents. On or about June 18, 2025, AV and W1 wheelchairs became entangled. W1 slapped AV in the head two times and hit/scratched another person. According to an investigation, AV and W1 have been in previous altercations, and AV is known to be an instigator, but this is not reflected in AV's care plan. The facility failed to provide services, to include implement interventions and / or follow W1 care plan and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
