Findings: According to the documentation, the facility failed to implement appropriate interventions to ensure the prevention of the Alleged Victim (AV) eloping out of the secure memory care. On or about June 25, 2024, at approximately 1430, the AV was found on the sidewalk walking by themselves without supervision. The AV has cognitive issues and is service planned to need an escort while outside the locked facility, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00831 $375.00 fine assessed
