Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) ambulation status with a walker. The facility received recommendations from physical therapy for the AV to use a walker to assist with unstable gait. Documentation shows the facility does not encourage the AV to use the recommended walker. On or about June 27, 2024, the AV was observed to have a bruise around their left eye, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
