Findings: The Alleged Victim (AV) relies on the facility for his/her care. Fall interventions documented in AV's care plan included ensuring AV wears supportive footwear removing tripping hazards and safety checks. According to an investigation, on or about June 24, 2024, AV fell while in the courtyard, resulting in skin tears to hand. AV had previous falls on or around 5/1/24, 5/29/24 with no service planned actions or fall interventions determined and documented for the resident. The facility failed to properly care plan, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
