Findings: Alleged Victim’s (AV’s) service plan dated June 20, 2024, states AV is to wear Geri-sleeves at night to prevent injury and that AV has thin skin which is fragile. Per a late entry for June 24, 2024, (Report Incident) it was entered stating the skin tears were discovered and staff stated that when AV was being put in bed on the 24th, the Geri sleeves were not applied, and AV’s left hand sustained a skin tear when the hand hit the bed rail. The facility failed to follow the care plan around the Geri sleeves for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-00114 $500.00 fine assessed
