Findings: The Alleged Victim (AV) relies on the facility for care and has an extensive fall history. During the month of February 2024, AV experienced approximately six (6) falls, some resulting in injury. According to an investigation, following a few of AV’s falls the facility failed to implement new, reasonable interventions and on or about February 22, 2024, AV fell and sustained skin tears above the left eyebrow, on the left hand, and on the right thumb. The facility failed to put effective, person-centered interventions in place after repeated falls which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: RCFCP25-00332 $3000.00 fine assessed
