Findings: The Alleged Victim (AV) relies on the facility for care and has an extensive fall history. During the month of March 2024, AV experienced approximately six (6) falls. According to an investigation, after a few of the falls, the facility failed to implement new, reasonable interventions and on or about March 24, 2024, AV fell and sustained a cut on their nose. The facility failed to put effective, person-centered interventions in place after repeated falls which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: RCFCP25-00332 $3000.00 fine assessed
