Findings: The Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) is known to have inappropriate behaviors. According to an investigation, on multiple occasions, W1 was found alone in a bedroom with AV, resulting in unreasonable discomfort. The facility failed to provide a safe environment to AV and appropriately care plan for W1’s known behaviors, which is a violation of resident rights, is neglect of care which constitutes abuse.
