Findings: The facility failed to provide appropriate services according to the Alleged Victim’s (AV) care needs for nail care. Based on facility documentation and interviews, AV has a diagnosis of a blood sugar disorder and requires full assistance for blood sugar disorder foot and toenail care. On or about July 3, 2024, AV was observed to have overgrown toenails with skin breakdown on the toes where the toenails rubbed and/or grown into the surrounding skin. AV suffered pain in the toes and AV toes hurt when AV tried to walk. The facility's failure to follow AV's care plan resulted in AV’s condition worsening causing further unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
