Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs, has cognitive deficits and a history of wandering and leaving the facility. AV's care planned intervention was for staff to provide 15-minute safety checks. According to an investigation, on or about July 8, 2024, AV was observed by staff to be "antsy" and was later discovered to have left the facility unsupervised. AV was found several hours later, several miles from the facility, in a ditch. As a result, AV was found with wounds to legs and evidence of heat exposure requiring hospitalization. The facility failed to ensure AV's service plan included interventions to address cognitive deficits and wandering behaviors to keep AV safe which is a violation of resident rights, is neglect of care which constitutes abuse.
