Findings: The facility failed to provide appropriate supervision to the Alleged Victim's (AV), according to his/her care needs and elopement risk. On or about June 29, 2024, at around 7pm, AV went into a section of the facility not meant for residents and AV went into this section of the facility without staff knowledge. Based on facility documentation and interviews, AV was observed wandering the facility prior to the incident. AV was found in the facility's sprinkler room and sustained a skin tear to AV's left elbow. The door to the sprinkler room was not properly secure at time of incident and AV was exposed to hazardous items not meant to be touched by a resident with cognitive deficits. The facility's failure to properly monitor AV resulted in unreasonable discomfort and exposed AV to potential risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
