Findings: The Alleged Victim (AV) has a history of falls and is care planned to be standby assist on all transfers, including toileting. On or about July 10, 2024, staff discovered that AV had went in to the common area bathroom independently. Facility staff left AV in the bathroom while they helped another resident, leaving AV alone. Staff assumed that AV had left the bathroom on AV's own, then heard AV yelling for help. AV had attempted to transfer themselves after toileting and had fell, resulting in swelling and a large hematoma on their lower abdomen and were transferred to the hospital for treatment. The facility failed to provide services, implement interventions and / or follow W1 care plan and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
