Findings: The Alleged Victim (AV) is known to wander into other resident’s rooms and is care planned to be redirected. On or about July 13, 2024, AV wandered into Witness 1’s (W1) room. W1 shoved AV out of W1’s room, resulting in a bruise on AV’s finger. The facility failed to follow AV’s care plan and/or properly plan care, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
