Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her needs, including incontinence care, and has a history of falls. According to an investigation, on or about July 13, 2024, AV was found on the floor with an injury to the head and near the eye. AV's service planned interventions were for staff to provide safety checks and toileting if awake in bed. Prior to the fall, AV had been approached for incontinence care but refused. AV was not re-approached for incontinence care the following shift util found on the floor. When AV was found with injuries to the head and eye, medical treatment was not provided timely. Emergency services were not called until several hours after the injuries were discovered. The facility failed to provide incontinence care, placing AV at risk of standing without assistance and having a fall and resulting in injury and bruising. The facility failed to provide timely care and medical treatment, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
