Findings: The Alleged Victim (AV) relies on the facility to ensure the AV's personal safety and wellbeing. Witness 1 (W1) was known to have sexual behaviors and inappropriate actions. According to an investigation, on or about July 12, 2024, W1 entered the AV’s room and made comments causing the AV to be afraid and refused to leave their room unattended. The facility failed to provide a safe environment to AV and follow W1’s behavioral care plans, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: RCFCP25-00624 $250.00 fine assessed
