Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. Witness 1 (W1) has known inappropriate behaviors. According to an investigation, on or about July 14, 2024, W1 entered the AV’s room, cornered the AV, undressed, got into the AV’s bed, and urinated on the AV’s belongings. The AV experienced a loss of personal dignity. The facility failed to provide a safe environment, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
