Findings: The Alleged Victim (AV) resides at the facility and relies on the facility to meet his/her daily needs. According to an investigation, on or about July 15, 2024, the Alleged Perpetrator 2 (AP2) slapped AV, pulled AV's hair and hit AV's head on the wall, resulting in AV feeling upset and fearful of retaliation. AP2's actions are a violation of resident's rights, are considered abuse and constitutes physical abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
