Findings: On or about July 15, 2024, the Alleged Victim (AV) was receiving shower cares from staff. AV's care plan was not up to date and listed AV as being independent and moderate assist with bathing. The facility's old system did not remove old interventions from the service plan, leaving staff confused on how to assist AV during showers. AV had a fall during the shower, and suffered a broken hip, requiring surgical intervention. Two care staff were present during the fall, one staff was placing items on the counter as the other staff were turning on the water and placing the shower chair for AV. The facility's failure to properly care plan so staff understood AV's needs is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00154 $1500.00 fine assessed
