Findings: The Alleged Victim (AV) relies on the facility for care and is a known fall risk. AV is documented as being an occasional assist with toileting and for staff monitoring for changes. AV is known to be forgetful. According to an investigation, staff were not provided with direction to check on AV throughout the day and/or frequency they should be monitoring AV. On or about July 17, 2024, AV had a fall off the toilet, resulting in contusions to AV’s face and forehead. The facility failed to provide appropriate services according to Alleged Victim’s needs, regarding care planning, safety checks and monitoring, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
