Findings: The Alleged Victim (AV) relies on the facility for his or her safety and is a documented elopement risk. On or about July 12, 2024, the Alleged Perpetrator 2 (AP2) left the secure area of the facility and did not ensure the doors closed and locked behind him/her. AV followed AP2 out the door and AV was alone in an unsecured area of the facility for approximately 1 minute, which provided AV access to the busy four (4) lane road right outside of the unlocked lobby doors. AP2 actions placed AV at risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
