Findings: On or about July 17, 2025, the Alleged Victim (AV) was in the spa bath without facility staff supervision. Witness #1 has a history of behaviors and altercations with other residents. (W1) tried to remove AV from the spa bath, as AV shouldn't have been in there alone. W1 grabbed a hold of AV, and gripped him/her with their fingernails, causing a scratch and torn skin to AV's arm. The facilities failure to ensure supervision of both W1 and AV, causing harm to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01410 $375.00 fine assessed
