Findings: According to the documentation, the facility failed to properly care plan appropriate interventions for the Alleged Victim’s (AV) known fall history. On or about July 7, 2024, the AV attempted to self-ambulate, fell, and sustained a small head wound. When staff assisted the resident to their feet and back into bed, the AV started complaining of extensive pain. Hospice was called and new medications were prescribed to mitigate the pain, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
