Findings: The Alleged Victim (AV) does have a history of falls and does have memory loss, impacting his/her ability to remember instructions given to him/her by staff to use his/her call light. On or about July 23, 2024, AV was found on the floor of his/her room by staff with his/her leg behind him/her, and complaining that his/her leg was numb. AV was transferred to the hospital and it was determined that AV suffered a fracture to his/her leg. The facility's failure to properly care plan to reduce AV's risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00180 $375.00 fine assessed
