Findings: According to the documentation, the facility failed to properly care plan appropriate interventions to ensure the Alleged Victim (AV) was safe to leave the facility on their own. AV’s current service plan did not determine the AV was an elopement risk, but labeled him/her that they had confusion, short term memory loss and was a risk of getting lost. On or about July 24, 2024, the AV was found by local law enforcement approximately a mile away from the facility and confused on where they were or how to get home. The failure to appropriately care plan for the AV’s ability to leave and return to the facility on their own is a violation of resident rights, is considered neglect of care and constitutes abuse.
